GodNation Foundation PMA Agreement
GodNation Foundation (GNF) is a Private Member Association (PMA) that includes GodNation Private Apothecary (GPA). Our focus is empowering wellness through health and renewal. We believe in the body’s innate power of healing therefore we focus on elevating its intrinsic abilities through our holistic specialized services for mental, physical, and spiritual rejuvenation. Being a member of this PMA allows you access to private services such as Consultations, Heavy Metal Cellular Detox, Full blood cleanse, Electromagnetic Therapy, Aromatherapy, GodNation Private Apothecary supplements etc. Your membership indicates acceptance and agreement to the following terms of services.
As member of GNF, I understand that I have the right to pursue various health practices and consult those whom I wish to achieve my goals for better health. I enter this PMA agreement on the behalf of any person I am legally responsible for which includes myself, minor dependents, or any adults for which I am legally responsible. This agreement is between me and GNF. I also understand that I am working with a representative who makes no claims of being a health practitioner; I do this of my own free will. I understand that no diagnosis or prescriptions will be given. I understand that GNF representatives are not medical doctors. I also understand that only medical doctors diagnose and prescribe treatments.
I willingly provide the representatives of GNF permission to openly discuss my personal health concerns as well as solutions with one another. GNF representatives may provide educational information including, but not limited to, biological, herbal botanicals, diet, nutrition, and other health information which does not include diagnosing, or prescribing treatment, and is not considered to be a substitute for medically necessary diagnostics. I fully agree to hold harmless any representative of GNF with whom I communicate in any manner and assume total liability for my actions. I understand and acknowledge that no representatives of GNF holds malpractice insurance and I agree not to file a malpractice lawsuit against any representative of GNF.
I attest that my actions with GNF are private, and I refuse to share with any state, or Federal Regulatory Board, FDA, FTC, Medicaid or any other insurance company without said permission. I agree to waive all HIPPA privacy rights and complaint processes. I understand that no doctor-patient relationship exists. I understand that it is my responsibility to educate myself on any advice or recommendations given to me by any representatives of GNF. Furthermore, the application of any learned techniques intended to remedy my personal health considerations without a medical doctor’s approval, is also of my own free will and is considered an act of exercising my constitutional rights. Purchasing GNF services/products as a recommendation for reversing any illnesses or deficiencies in the body and cells is only a recommendation. The recommendations given are suggestions and I (the client) must take responsibility of further educating myself in regard to my body’s health issues and the receiving of any proper medical care.
GNF products are considered to be supplements and not cures to any conditions or diseases. With proper diet these supplements have the ability to revitalize the immune, nervous, and lymphatic systems. The Food and Drug Administration has not evaluated statements made about specific supplements nor does the Food and Drug Administration recognize that any nutritional supplement is to be used as a cure for any condition. I (the member) will adhere to all compensation agreed to in correlation of services rendered and or products provided. I (the member) sign this PMA agreement without coercion in exchange for access to benefits that GodNation Foundation offers. By clicking yes, as my signature, I acknowledge that I have read, comprehend, and agree to the above terms stated.
